Lack of screening and preventive vaccines compliance for Sexual Transmitted Infections among young people who acquired HIV perinatally

Introdução

The challenges of care for young people who acquired HIV perinatally (YLHAP) go far beyond good adherence to antiretroviral therapy and virological control. Very little is known about routine screening and adherence to vaccines for sexually transmitted infections (STI) in this group.

Objetivo (s)

To analyze the prevalence of screening and vaccination against STIs among YLHAPs being followed up in a health service specializing in the care of people living with HIV.

Material e Métodos

In this cross-sectional study, we included the YLHAP being followed up in an outpatient clinic for adults with HIV/AIDS during the year 2023. Demographics, STI tests information (screening and serology) and data on STI vaccines (hepatitis A, B and HPV) were obtained from medical records in December 2023. We use descriptive analysis and Pearson's chi-square test to address associations, p<0.05 was considered statistically significant. There was approval from the research ethics committee. 

Resultados e Conclusão

88 YLHAP with a median 27 years (IQR 24.5; 30), 48.9% (95CI 38.4-59.4) female, all cis gender and 84.4% (95CI 74.3-91.1) heterosexual were included. Median T-CD4 count were 558.5 cells/mm3 (IQR 323.5; 771.5) and 75% (95CI 64.7-83.1) had undetectable (< 50 copies/ml) HIV viral load (HIV.VL). A half (51.1%, 95CI 40.6-61.6) had information about starting sexual activity. Prevalence for STI screening and STI vaccines coverage are at table 1. Peripheral TCD4 count were not associated with incomplete vaccination for hepatitis A (p=0.55), B (p=0.13) or HPV (p=0.90) and detectable HIV.VL was statistically associated only with an incomplete vaccination schedule for HPV (p=0.037). Vaccines and STI screening were suboptimally addressed in the YLHAP, with only vaccines received during childhood showing acceptable complete schedule rates. Health professionals should look beyond the challenging clinical management of HIV, adopting person-centered care that includes sexual life, to achieve greater success in STI prevention.
